mental health supportto join a person-centred service focused on empowering individuals to live fulfilling and independent lives within the community.
This role involves working closely with adults who may face a range of mental health challenges, offering emotional and practical support to help them develop life skills, maintain well-being, and achieve their personal goals.
Key Responsibilities
- Provide day-to-day support to individuals with mental health needs in accordance with their personalised care plans.
- Assist with daily living activities such as meal preparation, medication prompts, budgeting, and personal care when required.
- Support individuals to access community services, social activities, education, and employment opportunities.
- Build positive and trusting relationships with service users, encouraging independence and confidence.
- Monitor and report changes in mental health, behaviour, or support needs, ensuring appropriate action is taken.
- Maintain accurate and timely records of support provided, incidents, and progress notes.
- Work collaboratively with families, health professionals, and other stakeholders to provide consistent, high-quality care.
- Promote safeguarding and uphold the rights and dignity of all individuals in your care.
- Participate in regular team meetings, training sessions, and continuous professional development.
