About The Role

We are currently seeking compassionate and dedicatedSupport Workerswith experience inmental health supportto join a person-centred service focused on empowering individuals to live fulfilling and independent lives within the community.

This role involves working closely with adults who may face a range of mental health challenges, offering emotional and practical support to help them develop life skills, maintain well-being, and achieve their personal goals.

Key Responsibilities

  • Provide day-to-day support to individuals with mental health needs in accordance with their personalised care plans.
  • Assist with daily living activities such as meal preparation, medication prompts, budgeting, and personal care when required.
  • Support individuals to access community services, social activities, education, and employment opportunities.
  • Build positive and trusting relationships with service users, encouraging independence and confidence.
  • Monitor and report changes in mental health, behaviour, or support needs, ensuring appropriate action is taken.
  • Maintain accurate and timely records of support provided, incidents, and progress notes.
  • Work collaboratively with families, health professionals, and other stakeholders to provide consistent, high-quality care.
  • Promote safeguarding and uphold the rights and dignity of all individuals in your care.
  • Participate in regular team meetings, training sessions, and continuous professional development.
