Job description

The Crisis Resolution Home Treatment team is currently looking to recruit band 3 support workers. CRHT operates on a 24-hour, 7-day a week basis.

CRHT provides intensive home support to service users as an alternative to hospital admission. We work collaboratively with a variety of services to support service users at home. CRHT is based in Broadoak Unit and will cover Liverpool and Sefton areas.

The post holder will be expected to work on a rotational shift basis in accordance with service demands, including weekends and night shifts. They will assist with the continuity of care for service users within the appropriate care setting, primarily from the community environment.

The post holder will:

1. Provide support and time to an allocated group of service users, aiming to promote recovery predominantly within the community environment.
2. Facilitate and support the development of appropriate personal care.
3. Facilitate and support self-help, communication, and social skills acquisition.
4. Assist and promote independence with activities of daily living.
5. Facilitate a service user's recovery by assisting them to participate in social, vocational, recreational, and leisure activities within the appropriate setting.
6. Promote social inclusion through developing opportunities in areas such as employment, volunteering, and education.
7. Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff.
8. Participate in regular supervision, engage in reflective practice, and be committed to continued development.
9. Provide information to enable the effectiveness of care to be evaluated.
10. Ensure appropriate records are maintained under the direction of the qualified practitioner.
11. Contribute to the involvement of service users and carers/relatives in the delivery of care.
12. Provide reassurance and support to service users and carers/family.
13. Work collaboratively within the care team to ensure that service users' needs are met.
14. Recognise and respond appropriately to urgent and emergency situations.
15. Contribute to the development of services.
16. Contribute to the effective and efficient use of resources.
17. Help to identify early signs of relapse as described within the care plan and alert the appropriate staff involved in the service user's care.
18. Understand and adhere to Trust policies, procedures, and clinical guidelines.
19. Report any concern regarding service user care to the qualified practitioner.
20. Contribute to service improvement initiatives to enhance the quality of service user care.
21. Participate in service user and public involvement activities.
22. Support individuals' equality, diversity, and rights.
23. Participate in Serious Untoward Incident investigations/reviews as required.
