Role Overview

As a Support Officer you will join our friendly andhard-working team in supporting customers living in supported accommodationservice,enabling customers to sustain their tenancies by promotingindependence, freedom and choice.
Key responsibilities
- Assess referrals and engage with prospective clients using a person-centered approach
- Key to this role will be liaising with health professionals,other agencies and colleagues as appropriate
- Agree and review client Independent Living Plans and Risk Assessments on aregular basis, deliver contracted hours of support to achieve the requiredoutcomes
- Engage in continuous professional development and reflective practice
- Forge and maintain links with partner agencies and other key stakeholders asappropriate to ensure that good communication and co-operation is maintained
- Report any concerns relating to safeguarding of vulnerable people using thecorrect procedures, attending case conferences and other meetings as required
- Ensure that all relevant documentation and records are completed andsubmitted in an accurate and timely manner
- Ensure that prospective and current clients are fully aware ofthe support available, their financial obligations, rights and responsibilitiesunder their tenancy/occupation arrangements and all related procedures
About you
- Proven experience of working in a customer-facing role,motivating complex and diverse client groups to sustain housing, health and/orwork outcomes
- An understanding to motivate people who have complex and diverse issues,including alcohol or substance misuse, mental health trauma and behaviouralchallenges
- Previous experience working within housing would be beneficial,but not essential
- Strong understanding of safeguarding
