Job Description - Support Coordinator (25828)

Support Coordinator (Lead Mental Health Support Worker)

Location: Farnborough, North Camp

Type: Permanent, part-time (30 hours per week), working on a rota pattern

Salary: £13.62 per hour (£25,838 per annum, pro rata) plus benefits including Health Cash Plan

About the Role

Join our team as a Lead Support Worker, where you'll support customers with complex mental health needs to develop skills for independent living. Make a positive impact daily by helping customers achieve their goals in a caring environment.

Typical Day
  • Supporting adults with mental health needs and secondary needs such as substance misuse, physical disability, learning disability, and autism.
  • Starting shifts with handovers to receive updates on customers' needs and risks.
  • Managing your own diary to conduct support sessions on activities like cooking, shopping, budgeting, maintaining tenancies, and transitioning to independent living.
  • Carrying out risk assessments, support planning, and reviews.
  • Engaging in casual chats, collaborating with colleagues and external agencies.
What You Bring
  • Passion for supporting customers to live their best lives with attention to detail.
  • Experience in creating person-centred support plans and supporting colleagues.
  • Experience in coordinating and assessing customer referrals.
  • Ability to work independently, stay calm under pressure, and be resilient.

Job Details
