Job description

Change Grow Live is one of the UK's leading health and social care charities, and the Substance Misuse Nurse role is a varied one. You will support individuals with complex issues related to the impact of substance use on their physical and mental health.

Nurses are vital members of our multidisciplinary teams, providing specialized clinical interventions. Every individual who accesses our services is treated as a whole person, so understanding the importance of a holistic approach, being curious, a good listener, clinically robust, and flexible are essential skills.

Main duties of the job

Undertake physical and mental health assessments.

Refer or signpost to primary or secondary healthcare services for identified issues.

Conduct alcohol assessments to determine safe detoxification plans.

Support community alcohol detoxification and opioid detoxification processes.

Provide education, health promotion, BBV screening, and vaccinations for Hepatitis A/B.

Work within and support care pathways for BBV-infected individuals.

Perform phlebotomy as needed and interpret blood results to inform treatment decisions.

Refer for specialist inpatient services when necessary and liaise with hospital staff.

Contribute to Naloxone training and ensure availability of naloxone for individuals at risk.

Be proficient in basic life support, defibrillation, and emergency management.

Adhere to legislation regarding PGDs and support supervision of HCAs, students, and volunteers.

Job responsibilities

Main purpose of the role

As directed by the Lead/Cluster Lead Nurse, you will be an integral part of a multidisciplinary team providing specialist interventions.

All staff are expected to uphold professional standards, promote good practice, and safeguard vulnerable groups.

Reporting and working relationships

Report directly to the Lead/Cluster Lead Nurse. Reporting lines may evolve over time.

Role-specific responsibilities

Clinical Practice

Conduct health assessments, signpost or refer as appropriate, support detoxification processes, and deliver health education and BBV services.

Assist with phlebotomy, interpret blood results, and refer to specialist services when needed.

Support harm reduction strategies, including naloxone training and wound care.

Manage emergencies with proficiency in life support techniques and adhere to PGD legislation.

Communication

Build therapeutic relationships, ensure clear communication across teams, and maintain current knowledge of interventions.

Participate in meetings and contribute to ongoing professional development.

Delivering a Quality Service

Ensure high standards of service delivery aligned with guidelines and best practices.

Leadership

Provide clinical leadership and supervision to team members, fostering a supportive environment.

Team Working

Collaborate with colleagues to ensure effective service delivery, including transfer assessments and prescribing support.

Contribute to service performance and develop community detoxification plans.

Managing Information

Accurately record and manage client data, ensuring compliance with contractual and administrative requirements.
