A health and social care organization in Doncaster is seeking a Substance Misuse Case Manager to work with individuals affected by substance use issues. The role involves delivering personalized case management and psychosocial support while collaborating with various agencies to ensure holistic care. Candidates should have relevant degrees or diplomas in social work, psychology or health, along with experience in substance misuse services.

The Aspire Community Teams work with a range of complex individuals experiencing problems with their substance use, delivering case management, 1-1 psychosocial and harm reduction interventions, as well as supporting our clinical team in monitoring any prescribing interventions required. The teams work in partnership with a range of partner agencies to ensure holistic care packages which support longer term recovery. To ensure easy access to service provision the teams work in local hub venues.

Aspire is a partnership between ADS and Rotherham,Doncaster and South Humber NHS Foundation Trust (RDaSH).
