Main duties of the job

The Aspire Community Teams work with individuals experiencing complex substance use issues, providing case management, 1-1 psychosocial and harm reduction interventions, and supporting clinical teams with prescribing monitoring. They collaborate with partner agencies to deliver holistic care packages supporting long-term recovery. The teams operate from accessible local hub venues.

Aspire is a partnership between ADS and Rotherham, Doncaster and South Humber NHS Foundation Trust (RDaSH). It combines NHS expertise with voluntary sector strengths, working closely with recovery communities to provide flexible, responsive services aimed at achieving the best outcomes for service users and the community.
