Overview

Are you looking for the next steps in your nursing career as a Specialist Practitioner - District Nurse? Are you passionate about making a difference to patients' lives? If yes, there couldn't be a more exciting time to join us. You will be based within one of our community nursing teams who provide care to people in their own homes and have access to our Community Matrons and Specialist Nursing Services in addition to our Rapid Response and therapy teams. We empower our staff to work autonomously and with flexibility by providing state-of-the-art tablets for writing notes on the go electronically in real time. In this role, you will care for patients in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work.

About the opportunity

HCRG Care Group (North Kent) is inviting applications to undertake a 1-year full time Specialist Practitioner Award in District Nursing commencing January 2026. This challenging course consists of 50% taught sessions at the University of Greenwich and 50% work-based placements. Successful applicants will be employed on a full-time fixed term 12-month contract, receiving a salary and full funding for the Specialist District Nursing programme at the University of Greenwich. There will also be the opportunity to join us as one of our substantive colleagues at the end of the course and ongoing support to complete achievement of the Non-Medical Prescribing.

Responsibilities
  • Provide community nursing care within patients\' homes as part of a multidisciplinary team.
  • Collaborate with Community Matrons and Specialist Nursing Services, Rapid Response, and therapy teams.
  • Deliver high service standards, share best practice, and actively improve ways of working.
  • Operate with autonomy and flexibility, using electronic notes written on the go via tablets.
Qualifications
  • Be currently registered with the Nursing and Midwifery Council (NMC) as a Nurse
  • Have at least 2 years post-registration experience within community nursing
  • Hold a relevant degree, normally at 2:2 or above (at least 120 credits at level 6)
  • Show evidence of competence, experience, or academic ability to study at the level required for the programme
