Job summary

Chest Heart & Stroke Scotland (CHSS) is recruiting a Stroke Nurse to provide specialist assessment, advice and support to people affected by stroke and TIA, helping them maximise independence and personal goals.

Based across NHS Borders in a hybrid role, you will manage a community caseload, deliver person-centred support by phone and in person, and work with health and social care partners to improve services. Previous community nursing experience and a passion for stroke care are essential.

Qualifications

• Registered Nurse with strong clinical, communication and organisational skills.
• Willingness to travel across NHS Borders for community visits.
• Experience in community settings and ability to work independently.

Responsibilities

• Provide specialist assessment, advice and support to individuals and families following stroke.
• Manage a community caseload and deliver person-centred support.
• Collaborate with health and social care partners and contribute to service development.
