Job summary

Chest Heart & Stroke Scotland (CHSS) is recruiting a Stroke Nurse to provide specialist assessment, advice and support to people affected by stroke and TIA, helping them maximise independence and personal goals.

Based across NHS Borders in a hybrid role, you will manage a community caseload, deliver person-centred support by phone and in person, and work with health and social care partners to improve services. Previous community nursing experience and a passion for stroke care are essential.

• Registered Nurse with strong clinical, communication and organisational skills.
• Willingness to travel across NHS Borders for community visits.
• Experience in community settings and ability to work independently.

Every day, people across Scotland are adjusting to life following a diagnosis of a chest or heart condition, after experiencing a stroke or dealing with Long Covid. At Chest Heart & Stroke Scotland (CHSS), Scotland's leading health charity, we're working to ensure there is No Life Half Lived, and you could play a vital role in making that happen.

We provide a range of services, notably through our Community Healthcare Support Service, which delivers supported self-management and recovery services across Scotland.

By joining CHSS as a Stroke Nurse, you’ll have a real opportunity to help people move beyond simply surviving, towards managing their health, regaining confidence and independence and connecting with their communities.

About The Role

Join Chest Heart & Stroke Scotland's Stroke Nurse Service and make a meaningful difference to people affected by stroke and TIA. You will provide specialist assessment, advice and support to individuals and families, helping them manage their condition, maximise independence and achieve personal goals following stroke.

Working across NHS Borders, you will manage a community caseload, deliver person-centred support through a range of contact methods, collaborate with health and social care partners, and contribute to service development and quality improvement activities.

We are looking for a Registered Nurse with strong clinical, communication and organisational skills, a passion for improving outcomes for people affected by stroke, and the ability to work independently within a community setting.

Hybrid working role, primarily home-based, with travel required across NHS Borders for community visits, meetings and NHS premises. Occasional travel to other locations may also be required to meet service and organisational needs.
