Job Description:

This is a fixed-term contract until 30 September 2024. Our services are project-funded, and we currently have funding for this service until that date.

We are seeking an innovative, passionate, and professional individual with excellent interpersonal and organizational skills to join our team delivering the Stroke Recovery Service in Norfolk and Waveney. Due to increased local funding supporting stroke services across Norfolk and Waveney, we are recruiting five new support coordinators to help implement our strategy and make a meaningful difference.

Reporting to the Service Delivery Coach in the East of England, you will support stroke survivors in identifying their goals. Using strong assessment skills and a person-centered approach, you will enable them to maximize their recovery and improve communication.

Key Responsibilities:
  • Supporting new stroke survivors and their carers from hospital discharge into the community.
  • Providing personalized information, advice, and support.
  • Enabling stroke survivors to make informed lifestyle changes to prevent further strokes.
  • Collaborating with other health and social care professionals across the stroke pathway to ensure high-quality support.
Requirements:
  • Experience or background in a caring profession, ideally supporting people with disabilities.
  • Excellent IT skills and the ability to maintain accurate records.
  • Alignment with the values of the Stroke Association.
  • A flexible approach and effective caseload management skills.

This role requires frequent travel across a large geographical area to visit people at home and in community settings across Norfolk and Waveney. Candidates must demonstrate how they can meet this travel requirement.
