Job summary

Cardiff Council is seeking an ambitious Operational Manager for Locality and Care to shape and lead services that keep adults independent, safe and connected in their communities. The role covers locality-based care across Cardiff, with emphasis on prevention, reablement and person-centred support, and partnership working with health, housing and third sector organisations.

Qualifications

• Significant experience in strategic and operational leadership of locality-based care services.
• Strong understanding of prevention, reablement and person-centred support.
• Proven ability to lead change, improve performance and develop evidence-based proposals.
• Excellent partnership, communication and stakeholder management skills.
• Ability to inspire and develop high-performing teams.
• Welsh language ability would be an advantage.

Responsibilities

• Provide strategic and operational leadership of locality-based care across Cardiff.
• Oversee preventative, community-based and integrated care to maintain independence.
• Lead First Point of Contact and wellbeing services with effective information and assistance.
• Direct services supporting unpaid carers through a dedicated team.
• Oversee the Integrated Discharge Hub to support timely hospital discharge.
• Manage Telecare, Meals on Wheels and community day opportunities.
• Foster partnership working with health, housing, care providers and third sector.
• Drive continuous improvement, performance, and financial sustainability.
