Main duties of the jobWorking in close collaboration with Integrated Care Systems (ICSs), NHS Trusts, Local Authorities, and third-sector partners, you will be at the forefront of embedding CHPs LIFT estate into long-term service models and delivering our national PACE (Productivity Acceleration in Community Estate) programme.
