Job summary

ABL Health in Sefton is seeking a Stop Smoking Practitioner (training provided) to deliver evidence-based smoking cessation services across community venues, GP practices and workplaces. The role requires flexibility to work evenings and travel between sites.

You will work under supervision, providing one-to-one and group support, use NCSCT Standard Treatment Programme, and contribute to reducing health inequalities in Sefton.

Qualifications

• Experience in delivering smoking cessation services or willingness to learn.
• Ability to work across multiple venues and with diverse client groups.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver evidence-based stop smoking interventions at community venues, GP practices and workplaces.
• Support referral pathways and ensure timely referrals from hospital settings when appropriate.
• Provide education, motivational support and goal setting to clients to achieve quit attempts.

Role Purpose:

This is a highly rewarding role that offers the opportunity to make a meaningful difference to people’s lives. The Specialist Stop Smoking Practitioner will support the delivery of evidence-based stop smoking services aimed at reducing health inequalities across the borough of Sefton. The role will include delivering stop smoking and tobacco dependence interventions throughout multiple venues across Sefton including Community centres, GP practices and workplaces. The postholder must be able to work flexibly, independently, and collaboratively as part of a team and be willing to work one evening per week to meet the needs of the service.

Part of this role will also involve occasional work with hospital settings. This will include reviewing and updating referral pathways to make sure patients are referred to the service promptly when they are discharged from hospital.

Training will be given to ensure staff / applicants are equipped to deliver effective services.

Key Duties and Responsibilities

Working under the line management of the Head of Service, you will play a key role in delivering and developing high-quality smoking cessation services across the local community. You will work collaboratively with partner organisations and services to promote the service, increase referrals, and embed smoking cessation into routine client and patient care. You will motivate and empower individuals to make positive, sustainable changes, providing evidence-based stop smoking interventions through a range of delivery methods, including one-to-one appointments, drop-in and group sessions, telephone and online support, in line with the NCSCT Standard Treatment Programme. You will provide evidence-based education and ongoing motivational support, work with clients to set realistic goals, and lead peer support sessions for successful quitters to help prevent relapse. The role will also involve interpreting health, equality and service data to understand local needs and inform service delivery, while ensuring a high-quality, efficient and cost-effective service across diverse community settings. You will be responsible for maintaining accurate, up-to-date client and service records in line with ABL Health documentation policies and will need to manage competing priorities effectively, using your own initiative and professional judgement to meet agreed timescales. Skills and Competencies Required

· Be an experienced Specialist Stop Smoking Practitioner within the community. (Training will be given)

· Experience of working with, evolving and developing services

· Positive and proactive outlook with the ability to motivate, engage and support others to achieve the highest standards and outcomes.

· Adept in communication; A strong communicator who adapts the message to fit the needs of the audience including clients, staff, and volunteers at all levels.

· Build strong relationships, foster trust and co-operation among colleagues, stakeholders throughout community settings.

· Quality driven; you naturally seek high standards and actively seek to improve them.

· Value and remain open to new ideas and perspectives.

· Due to the nature of this role and potential for regular travel across the Borough, we require applicants to have a driving licence and access to a vehicle for work purposes.
