Job summary

Active Care Group in Birmingham is seeking a Staff Nurse for our Hunters Moor neurological rehabilitation service. The role involves working as part of a multidisciplinary team to deliver individual care plans and ensure safe, person-centred care for adults with neurological conditions.

You will participate in assessments, support care planning and communicate with families and carers. The position requires an active NMC PIN and the right to work in the UK, with rotas including days, nights and

Qualifications

• Must have NMC registered pin (RN).
• Legal right to work in the UK.
• Experience in neurological rehabilitation is desirable.

Responsibilities

• Be an active participant in the multidisciplinary team, formulating care plans and carrying out assessments.
• Assess and manage risks identified through patient assessments in conjunction with other health professionals.
• Regularly review risk factors and modify care plans accordingly with the care team.
• Liaise with carers, relatives and other involved parties to deliver integrated patient care.
• Maintain professional knowledge/portfolio and pursue relevant training to enhance care quality.
