A leading hospice in the UK seeks a nurse practitioner for its Night Response team, specializing in palliative care. This role involves providing care to terminally ill patients in the community, requiring effective communication and teamwork skills. We are looking for a nurse practitioner, experienced in palliative care or a community setting, to join our Night Response team. This post will enable you to develop your skills in caring for patients towards the end of life and supporting them to be cared for in the place of their choice.
The postholder will work flexibly on the Night Response Rota, taking calls coming into the Hospice overnight, providing generic advice, signposting, and redirecting calls to other services as appropriate. You will liaise with other staff on the Inpatient Unit to sustain and develop the relationship between Princess Alice Hospice and the patients/families receiving palliative care support.
Experience in caring for patients who are terminally ill is essential. Applicants will need their own car and a full driving license, as you will be visiting patients in the community.

Responsibilities include carrying out personal care, activities of daily living, delegated nursing activities such as catheter and stoma care, and assisting with maintaining a homely atmosphere in the patient's home. You will communicate effectively with patients and their families, follow hospice policies, and work as part of the team to uphold high standards of care.
The post may require supporting the Inpatient Unit during capacity issues, under supervision.
