Job description

Job summary

The neonatal outreach team is a small and friendly team working between the neonatal unit and the community, supporting parents to take their children home when additional input is required.

We are seeking a motivated and dynamic band 5 nurse to join our team.

The team collaborates closely with the larger neonatal unit to improve the discharge process. University Hospital Tees offers numerous supportive and educational opportunities to help staff develop into well-rounded team members.

The postholder will be part of the 7-day Neonatal Outreach Team, providing high-quality nursing care for neonates and their families preparing for discharge from NICU, and supporting them at home post-discharge. Responsibilities include discharge planning with the multidisciplinary team, family education, and ensuring families are well-prepared for home care.

Additional duties involve supporting families on the Transitional Care Unit, liaising with community teams, and caring for families at home. The successful candidate must have access to a vehicle for work purposes.

This role will support new initiatives such as home phototherapy and antibiotics, ensuring timely and safe discharge, family education, and involvement in community clinics. The nurse will act as an advocate for families.
