Job summary

Royal United Hospitals Bath NHS Foundation Trust is looking for a committed nurse to work as a named nurse or key worker, delivering patient‑focused care under the NMC Code of Professional Conduct.

You will assess care needs, plan and evaluate programmes, coordinate discharge arrangements, educate patients and carers, and contribute to teaching and supervision within a supportive, multidisciplinary team.

Qualifications

• You’ll need to be able to understand and communicate both verbally and in writing in English.
• You’ll need to understand what these mean as you’ll be documenting the results in patient notes and communicating to your team.
• You’ll need to have numerical and analytic skills to do patient observations.

Responsibilities

• Acting as a Named Nurse, to maintain accountability for assessing, planning, implementing and evaluating programmes of care within the framework of team nursing.
• Co-ordinate the patients’ discharge arrangements in accordance with RUH Discharge Policy.
• Promote a patient focused approach to care in collaboration with all relevant health professionals.

• Registered with Nursing & Midwifery Council
