Job description

We are seeking a highly motivated and adaptable individual to join our team at the Discharge Lounge, Integrated Hub, Huddersfield Royal Infirmary.

The successful candidate should be able to use their own initiative and work effectively within a team.

The discharge lounge is a busy, proactive, and ever-changing environment that is diverse and challenging. It offers a multidisciplinary working environment and a friendly place to work.

Key Responsibilities:
  1. Transferring and accepting patients from various ward areas to the discharge lounge and integrated hub.
  2. Coordinating discharge planning, liaising with care providers, care facilities, and family members. Recognizing the need for further community referrals to safeguard patient safety and ensure effective discharge.
  3. Completing patient education regarding care providers, discharge plans, medication management, and follow-up arrangements.
  4. Maintaining effective communication and documentation on the electronic patient record.
  5. Addressing hygiene, nutrition, hydration, elimination, and mobility needs during patient care, offering comfort, compassion, and reassurance.

We employ over 6,500 staff delivering compassionate care across our hospitals, community sites, health centres, and patients’ homes. We also have nearly 150 volunteers.

Our services include urgent and emergency care, medical, surgical, maternity, gynaecology, critical care, children’s and young people’s services, end-of-life care, and outpatient and diagnostic imaging services.

We also provide community health services, including sexual health services at Calderdale Royal and local health centres like Todmorden Health Centre and Broad Street Plaza.
