A leading healthcare provider in Chelmsford seeks a professional Staff Nurse for their Cardiac Rehabilitation team. You will support patients through their rehabilitation journey and work closely with multidisciplinary teams to enhance patient care. The position requires Immediate Life Support training and relevant experience in cardiology or rehabilitation. The role offers a salary of £31,049 per year.

Our Cardiac Rehabilitation service at Broomfield Hospital is a well‑established team covering Broomfield Hospital and the mid‑Essex region, and we have a wonderful opportunity for a professional, confident Staff Nurse to join its small supportive team.
Your role will assist in supporting the current high‑quality Cardiac Rehabilitation service and the patient choice initiative, its future development including physical, educational, emotional, and psychological support of patients, relatives and carers enabling them to live with their disease, treatment and the implications thereafter.
You will be involved in all phases of the cardiac rehabilitation process, liaising effectively to establish and maintain professional relationships with the multi‑disciplinary team, agencies, and organisations and covering both inpatients and outpatients working across all sites (Broomfield/Maldon/Witham/Braintree districts).
As an information resource for patients, you will establish good relationships with patients and their families through all phases of cardiac rehabilitation and patient choice initiative, help with Phase II telephone follow‑up calls, ensure accurate documentation, and compile comprehensive and accurate advice and discharge letters to GPs and other multi‑disciplinary staff.
You will maintain a safe environment for patients attending cardiac rehabilitation as per risk assessments and protocols, assess, plan, deliver and evaluate patients’ care utilising an individual holistic approach, monitor all relevant NICE Guidelines and assist in the planning of discharge from rehabilitation of cardiac patients including future goal setting, implementation of secondary prevention issues and informing primary care clinicians of the patient’s status and future needs.
