An exciting opportunity hasarisen for a Staff Nurse to join our growing team on a full time (34.5 or 37.5 hours available). Wakefield Hospice is looking for those of you who have the passion anddrive to deliver high quality specialists palliative care in our inpatientunit, have effective communication skills and at least 12 months UK post-registrationexperience in working in community care settings. You will be provided with afull induction programme, a buddy/mentor, and general support to allow you tokeep developing your skills and knowledge in specialist palliative care whilstworking in a high quality clinical environment.

The successful candidate must demonstrate compassion,commitment, caring, competence, quality and safety in delivering person centredcare.

Main duties of the job

Wakefield Hospice provides specialistpalliative and end of life services 24 hours a day, 7 days a week. You will be required to work avariety of shift patterns across 24hrs, including rotation to day and nightshifts (and on occasion to support the Day Therapy Services) to:

Deliverhigh quality Specialist Palliative Care and End of Life Nursing as part of theInpatient Unit multi professional team.

Toassess, plan, implement and evaluate care for a named group of patientsensuring a holistic approach that is sensitive and responsive to patient andfamily's needs.

Todemonstrate compassion, commitment, caring, competence, quality and safety indelivering person centred care.

Tobe responsible for the coordination and smooth running of a shift whenrequired, ensuring it is well organised and safe.

Asa registered, accountable and autonomous nurse act in the best interests ofpatients at all times, taking personal responsibility for own actions anddecisions.

Wakefield Hospice, is committed to providing the highest level of symptom management and care for people who have advanced active, progressive and life threatening illness.

Wakefield Hospice is a purpose-built specialist palliative care unit that opened in April 1990 providing 8 single ensuite bedrooms and 2 four bed ward areas, a day therapy unit, bereavement support and education. Over the years the day care unit developed initially into a day therapy unit and ultimately into a drop-In centre providing therapies, information and care for patients, carers and bereaved families.

Job responsibilities

Clinical

Assess, plan, implementand evaluate person centred nursing care for a named group of patients incompliance with hospice policy and protocols.

Undertake holisticassessment of patients, families and carers, including their emotional,physical, spiritual, practical, welfare rights and as appropriate pre and postbereavement needs. Ensuring thatpatients, their families and carers are involved in their plans of care.

Actively support andplan the discharge planning and process.

To develop, update,monitor and evaluate patient care plans ensuring they are person centred andsupport individual needs.

To demonstrate theskills and knowledge that enable the delivery of basic and complex specialistnursing care interventions that are responsive to individual need.

Have knowledge ofcommon symptoms, anticipate change and evaluative interventions.

To communicateprofessionally, sensitively and effectively with patients, carers, families,the multi professional team and all staff within the hospice.

To deliver concise andaccurate patient presentation in daily hand overs and to attend and participatethe weekly MDT meeting in the absence of senior nursing team if requested.

In the role of wardcoordinator be responsible taking a nursing lead on admission and dischargeplanning as required. Take day to dayresponsibility for planning and organisation of the shift and prioritiseeffectively according to patient need, safety and staffing.

To communicateeffectively with own team and maintain effective communication between the MDTand external agencies.

To communicate highlysensitive information, ensuring patients, their families and carers aresuitably supported.

To respond to helplinecalls effectively and in a timely manner, including escalation to senior teammembers or appropriate signposting or advice is given and documented.

To promote and supportAdvance Care Planning.

Ensure accurate andtimely records of care (electronic and paper) are maintained, upholdingcompliance with information governance, being responsible for having a SystmOnecard on all shifts.

Follow all agreedpolicies for manual handling, infection control, health and safety andsecurity. Failure to adhere to these guidelines causing harm to a patient,family member or hospice employee could lead to disciplinary action.

Action the agreedpolicy in the event of a fire/fire drill.

Ensure the dignity,privacy and confidentiality of all patients and families according to Caldecottprinciples, hospice policy and guidelines.

Participate in audit and evaluation of clinicalpractice and lead on delegated areas of clinical development.

Actively promote and engage in reflection andclinical supervision

Actively promote a culture that supportseffective team work.

Support all students including nurse associatetrainees.
