Job summary

St Helens and Knowsley Teaching Hospitals NHS Trust is seeking a nurse to assess patients, plan and deliver care across changing health needs and rehabilitation in intermediate care settings.

You will supervise staff and contribute to health promotion, with responsibilities that may include task allocation and quality checks. This role supports patient-centered care within a collaborative team.

Responsibilities

• Provide a nursing assessment of patients/clients.
• Plan, develop and evaluate care programmes.
• Provide rehabilitation for Intermediate Care patients.
• Provide clinical supervision to other staff including students.
• May allocate routine work and check tasks of support staff.
• Provide health promotion information and advice.

Job description

Job Purpose The post holder is required to: Provide a nursing assessment of patients/clients; plan develop and/or implement and evaluate programmes of care for those with changing health needs. Provide rehabilitation for both step down and step up patients using a comprehensive skill set to support the needs of Intermediate Care patients. Provide clinical supervision to other staff including students May be responsible for routine work allocation and checking of tasks of support staff and/or less experienced staff.
