Exciting opportunities have arisen following a review of provision of mental health services for older people at West London NHS Trust. We are in the fortunate position of transforming and expanding our service to meet the needs of older people with mild to severe mental health needs. We are looking for dynamic innovative staff to be part of a team that will work therapeutically to support older patients in their mental health recovery in the community.

We are recruiting one Band 6 Nurse Practitioner and this post is based at Hammersmith and Fulham catchment area. Assessments and intervention are delivered at patients' homes therefore flexibility is important. Hammersmith has excellent public transport links therefore having a car or driving is not essential. Most of our clinicians use the bus on visits. All expenses will be refunded on a monthly basis.

Main duties of the job

The post is an ideal opportunity for an experienced Nurse to build upon and advance their clinical and professional skills and support their subordinates. The focus of the service will be to ensure that patients receive assessment and therapeutic intervention to maximise their independence, safety and wellbeing. The interventions will include education, individual work and group work and will involve working closely with patients, carers, statutory and voluntary agencies, within a multi-disciplinary team. They will hold their own allocated caseload of patients.

The post holder will be based in Hammersmith and Fulham Community Team and they will be undertaking home visits, meetings at the team base and other community sites.

Qualifications and Training
  • Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing
  • Evidence of continuous professional development
  • Community Nursing qualification
Experience
  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff.
  • Experience of managing a caseload.
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties.
  • Experience of working collaboratively with professionals.
  • Experience of conducting a range of clinical assessments including risk assessments.
  • Experience of partnership working within and across various statutory and non-statutory teams and agencies.
  • Experience in working with people with mental health functional issues, on the ward and in the community.
Knowledge
  • Understands the principles of recovery / person centered care and their application to risk management, positive risk taking, and strengths orientated approaches to care delivery.
  • Have an excellent understanding of assessment and management of risk.
  • Have an understanding of the CPA process.
  • Have a good clinical understanding of the physiology and pharmacology in relation to older people’s health.
  • Effective planning & organizational skills with an ability to prioritize own workload.
  • Understanding of NICE guidance relevant to older people.
  • Have a good knowledge of service objectives.
