Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Band 5 Registered Mental Health Nurse to join the Older People’s Mental Health Team in Newbury. The post is full-time, 37.5 hours, Monday to Friday, based at Hillcroft House on the West Berkshire Community Hospital site.

You will work within a supportive MDT, delivering high-quality, person-centred care to older adults and those living with dementia in the community.

Qualifications

  • Qualified RMN with current registration
  • Driving licence required
  • Ability to work within a multidisciplinary team

Responsibilities

  • Manage a designated caseload delivering person-centred care
  • Conduct comprehensive mental health assessments in community settings
  • Develop and review collaborative care plans with patients, carers and families
  • Deliver evidence-based interventions and monitor outcomes
  • Maintain accurate clinical records in line with policy and legislation
  • Participate in MDT discussions and care planning to support positive outcomes
  • Ensure safeguarding and professional standards are followed
  • Pursue CPD and maintain professional registration

Job description

We have an exciting opportunity for a Band 5 Registered Mental Health Nurse (RMN) to join our supportive and friendly Older People's Mental Health Team based within the Newbury locality. We welcome applications from clinicians who are passionate about working with older adults experiencing functional mental health difficulties, as well as people of all ages living with dementia, within a community setting. This is a full-time post based at Hillcroft House, on the West Berkshire Community Hospital site. You will be joining a well-established multidisciplinary team that values collaboration, professional development, and high-quality, person-centred care. We pride ourselves on creating a supportive working environment where colleagues work together to achieve the best possible outcomes for patients and their carers. This role offers an excellent opportunity to develop your skills and experience within a specialist community mental health service, while making a meaningful difference to the lives of people affected by mental health difficulties and dementia. The post is 37.5 hours per week, worked Monday to Friday, 9:00am to 5:00pm.

  • Manage & take responsibility for a designated caseload, ensuring the delivery of high-quality, person-centred care.
  • Undertake comprehensive mental health assessments in a variety of community settings, considering the individual's physical, psychological, social, cultural & spiritual needs.
  • Develop, implement & review collaborative care & treatment plans in partnership with patients, carers, families & other professionals.
  • Deliver evidence-based interventions & monitor their effectiveness, adapting care plans in response to changing needs & risks.
  • Maintain effective communication & collaborative working relationships with primary care services, local authority colleagues, voluntary organisations & other partner agencies to ensure coordinated care.
  • Actively contribute to multidisciplinary team discussions, care planning & decision-making to support positive outcomes for service users.
  • Maintain accurate, timely & high-quality clinical records in accordance with professional standards, Trust policies & legal requirements.
  • Ensure practice is delivered in line with Trust policies, professional codes of conduct, safeguarding procedures & relevant legislation.
  • Take responsibility for maintaining professional registration & continuing professional development, contributing to a culture of learning within the team.
  • Develop & maintain a specialist area of interest relevant to Older People's Mental Health Services by sharing of knowledge & best practice across the team.
RMN qualification
  • Have a passion for working with people of Older Age.
  • Hold a valid driving licence and have access to a vehicle for work purposes.
  • A desire to work and be part of a friendly and supportive team striving to offer the best service to our patients.
