Job summary

Active Care Group is seeking a Staff Nurse (Tracheostomy and Neurorehabilitation) at the Hothfield Brain Injury Rehabilitation Centre in Kent. You will act as Primary Nurse, coordinate nursing input, and contribute to MDT care plans for adults with brain injury or neurological conditions.

The role requires up-to-date tracheostomy and neurorehabilitation experience, NMC PIN, and a flexible 24-hour rota across days, nights and weekends.

Qualifications

• Current NMC Pin registration (RGN).
• Legal right to work in the U.K.

Responsibilities

• Acting as Primary Nurse with overall nursing input for residents.
• Contributing to MDT working, risk assessment, and ongoing clinical improvement.
• Providing written nursing reports for statutory and clinical reviews.
• Adhering to NMC Code of Professional Conduct as named nurse or key worker.
• Working as a key team member to help patients achieve planned outcomes.
• Ensuring comprehensive person-centred assessments for pathways and care plans.
• Taking charge of a designated clinical area.
• Maintaining high standard clinical documentation and patient notes.
• Providing a highly specialised range of clinical interventions.

We’d like you to join us as a Staff Nurse (Tracheostomy and Neurorehabilitation) at our Hothfield Brain Injury Rehabilitation Centre in Kent. At Hothfield Brain Injury Rehabilitation and Neurological Centre we look after adults with a brain injury or other neurological condition. What you’ll be working:

• 37.63 hours per week Rota'd Days/ Nights & Weekends

What you’ll be doing:

• Acting as Primary Nurse, taking overall responsibility for the nursing input residents receive, through nursing assessments and care planning.
• Contributing towards MDT working, risk assessment management and a programme of on-going clinical improvement.
• Providing written nursing reports for a range of statutory and clinical review processes.
• Work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker for a defined group of patients.
• Work and act as a key member of the team, supporting each patient with achieving their planned outcomes.
• To ensure a comprehensive assessment is completed for the patients/residents/clients to ensure a comprehensive person-centred assessment is completed for their pathway and care plan.
• Take charge of a designated clinical area.
• Ensure all clinical documentation is of a high standard and patient notes are maintained and filed in the appropriate way.
• Provide a highly specialised range of clinical interventions and knowledge of evidence based models of practice relevant to the patients’/residents’ needs.
• Working autonomously with confidence and competence to provide a highly specialised and safe service to the patient.
• Adopt a flexible approach to hours of duty, in order to meet the needs of the service over a 24 hour period.

What we would like from you:

• A current NMC Pin registration (RGN)
• A Legal right to work in the U.K.
• Up to date experience/knowledge of Tracheostomy care essential
• Up to date experience/knowledge in management of neuro conditions essential
• Experience of working within a neurorehabilitation setting
