SRA PCN Manager & Digital & Transformation Lead

We are seeking an experienced, forward-thinking leader totake on the role ofPCN Operations Manager and Digital &Transformation Leadfor Shropshire Rural Alliance PCN. This is an excitingopportunity to shape local health services, lead meaningful change, and work atthe forefront of innovation in primary care.

You will lead the operational delivery of key servicesacross the PCN, including Enhanced Access, ARRS workforce development, anddigital transformation programmes such as EMIS Hub, cloud telephony, anddigital triage. This role combines hands-on coordination with strategicleadership and system-level collaboration.

Based in Rural Shropshire and supported by a proactive teamand federation infrastructure, this role offers variety, autonomy, and impact.You will work closely with clinical leaders, practice managers, and widerstakeholders to improve patient outcomes, reduce inequalities, and future-prooflocal care.

Main duties of the job

This is a hands on leadership role at the heart of primarycare. You will be based across practices in SRA PCN, working with a wide rangeof clinical and non clinical teams to improve patient access, support ourstaff, and help services run smoothly every day.

This role offers real variety and impact. One day you couldbe building the Enhanced Access rota and supporting a new team member, the nextyou might be troubleshooting EMIS sessions, attending a system meeting, orsupporting a GP practice to improve their digital triage. No two days are thesame, and you will work with people across all levels of the health system.

About us

Our Health Partnership was set up by local GPs who arepassionate about providing high quality primary care and using their time andskills effectively to benefit patients.

We are currently a Primary Care Network made up of five GPpractices supporting approximately 30,000 patients in and around ruralShropshire. (Clive, Knockin, Pontesbury & Worthen, Shawbury and Westbury)

We are an integrated community-based team delivering a rangeof additional services such as extended hours appointments, paramedic homevisiting and first contact physiotherapists to name just a few. By workingtogether, we can proactively offer more effective and efficient services thatmeet our patient needs.

Job responsibilities

As PCN Operations Manager and Digital & Transformation Lead, you will lead the coordination and delivery of services across SRA PCN. You'll oversee Enhanced Access clinics, manage the deployment and performance of ARRS roles, and act as the strategic lead for digital programmes such as EMIS Hub, cloud telephony, and digital triage.

You'll manage and support staff across practices, lead on workforce planning and transformation, and ensure services meet contractual and regulatory requirements. The role involves strong partnership working with practice teams, ICB colleagues and system stakeholders.

This is a hands-on leadership role for someone who enjoys improving services, working with data, leading people, and using digital solutions to enhance access and patient care.

Main responsibilities

Enhanced Access and PCN services

  • Coordinate day to day running of Enhanced Access including clinic set up, session planning and rotas
  • Work with practices and ARRS staff to ensure service delivery is safe, consistent and patient centred
  • Support the development of PCN level services in line with national and local guidance
  • Maintain up to date operating procedures and templates to support delivery

Digital and transformation

  • Lead on implementation and management of EMIS Hub, digital triage, cloud telephony and related systems
  • Set up user access, troubleshoot basic issues and support adoption across practices
  • Use data and dashboards to monitor demand and inform planning decisions
  • Help embed new ways of working using technology to improve access and efficiency

Workforce coordination and team support

  • Line manage ARRS staff and PCN support team members
  • Support recruitment, onboarding, one to one conversations and team development
  • Help staff feel connected across sites and supported in their roles
  • Liaise with HR and finance colleagues to support retention and performance

Partnerships and communications

  • Act as a key link between practices, federation teams, ICB partners and system stakeholders
  • Represent the PCN at meetings and share updates across teams
  • Build strong working relationships and help practices collaborate to solve shared challenges

Monitoring and reporting

  • Support contract delivery and ensure compliance with key targets and outcomes
  • Keep service plans, action logs and reports up to date
  • Track service performance, budget usage and support funding claims

Other

  • Champion equality, inclusion, safeguarding and good governance across the PCN
  • Stay up to date with training and NHS developments
  • Travel between sites in the PCN area as required
Person Specification
Knowledge and Skills
  • Strong organisational skills including planning, prioritising, time management and record keeping.
  • Ability to work collaboratively, liaising with other stakeholders as needed for the collective benefit of patients including GPs, healthcare professionals and other practice staff.
  • Ability to prioritise own workload.
  • Computer efficient with the ability to apply effect IT systems.
  • Excellent written and verbal communication skills.
  • Excellent team working skills.
  • Ability to work independently, showing initiative.
  • Knowledge of Primary Care and how it operates.
  • Knowledge of Primary Care Networks and the Network Contract DES specifications.

Qualifications
  • Excellent understanding of NHS policy, PCN contracts, and primary care strategy
  • Relevant qualifications or certifications in digital or health informatics, such as Digital Health Leadership, NHS Digital Academy, or equivalent
  • Strong financial acumen with experience managing budgets and overseeing performance reporting
  • Masters degree in healthcare management, public health, digital innovation, or related discipline

  • Meet DBS reference standards and criminal records checks.
  • Access to own transport / full clean driving license.
  • Ability to travel across the locality if needed.
Experience
  • Significant experience at a senior level in health or care operations, transformation, or commissioning
  • Demonstrable people management experience, including line management, team development, and performance oversight
  • Excellent understanding of NHS policy, PCN contracts, and primary care strategy
  • Experience in delivering digital transformation programmes and leading adoption of new technologies in a healthcare setting
  • Strong financial acumen with experience managing budgets and overseeing performance reporting
  • Excellent stakeholder management and communication skills with experience of system-level collaboration
  • Proven ability to manage multiple priorities, adapt to change, and deliver results independently and as part of a team
  • Experience leading complex change across multiple organisations or systems
  • Familiarity with EMIS Web, EMIS Hub, and digital tools used in primary care transformation
