Job summary

Mersey West Lancashire NHS Trust is seeking a Spinal Acute Outreach Practitioner to connect NWRSIC with patients and multi-professional teams across the region. You will provide expert advice on spinal cord injury management and contribute to education, service development, and bed capacity planning.

You will lead patient pathways, support nursing staff, and promote evidence-based practice within the Trust. A key role includes mentoring and facilitating high-quality care across sites.

Qualifications

• First level registered nurse / HCPC registered Allied Health Professional
• Degree in health related area
• Teaching/mentoring qualification e.g. Level 6: Mentoring in Practice
• Advanced communications skills
• Recognised counselling qualification e.g. Level 3: Diploma in Counselling Skills

Responsibilities

• Provide expert information, advice and support for spinal cord injury patients and carers.
• Lead on specialist nursing practice and pathway development for spinal cord injury services.
• Act as Key Worker for patients undergoing investigations and treatment.
• Develop, implement and monitor nursing practice standards and care pathways.
• Coordinate liaison across Multidisciplinary teams to ensure timely care and discharge planning.
