Job description

The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care, and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home.

JOB SUMMARY:

We are hiring dynamic, passionate Speech Language Pathologists (SLPs) who want to be part of transformational change in home and community care by providing care in a truly client-centered approach. You are excited about working in an environment that prioritizes the care team, offering high professional autonomy and responsibility. You will be part of a diverse team providing services to clients in the community, engaging in joint care planning focused on client-centered goals across physical, cognitive, social, emotional, and spiritual domains to create holistic care plans. Each client is assigned a primary care clinician who leads the care planning and facilitates team efforts to meet client goals. As part of the SE Transitions team, you will be accountable for the care and services you provide and the outcomes your clients achieve.

JOB RESPONSIBILITIES:

  • Conduct assessments
  • Formulate goals and recommendations
  • Plan and implement client-centered, goal-directed treatment programs
  • Experience in managing patients in acute/rehab or community-based rehabilitation preferred
  • Work effectively with rehab assistants and interdisciplinary teams
  • Knowledge of community resources is an asset
  • Maintain effective interpersonal, organizational, and communication skills
  • Maintain a positive work and attendance record
  • Adapt to new computer applications
  • This role involves a hybrid work approach, combining remote work with client visits in acute/rehab or community settings

A vehicle, valid driver’s license, and ability to travel are required.
