Job summary
The Older People's Mental Health Community Mental Health Teamwork closely with GPs to ensure early identification of patients with mental health needs and facilitate timely referrals. Assessments will be provided by a multi-disciplinary team, ensuring carers and family are involved at the initial assessment and in care planning.
Through early diagnosis of mental disorders the delivery of evidence based therapeutic initial interventions (pharmacological and psychological) will be facilitated in a timely fashion.
Support and information delivered in a person-centered way after the diagnosis to ensure the person with dementia and their carer are adjusting to the diagnosis and its consequences and are engaging in personal care plans and understand how to access other assistance.
Continuity of care across the pathway and integration at patient level with other care providers
Delivery of care in a considered, timely and co-ordinated manner
This integrated approach to the care of the elderly and the support of their carers, will be provided in partnership with local healthcare, social care and voluntary/ independent organisations.
