Main duties of the job
The post holder will be a Specialty Doctor as part of the HomeFirst Team covering the following clinical areas:
- Supporting the UCR and Virtual Ward services
- Provide middle grade support to the Mary Seacole Unit
The post holder will join our exciting and rapidly expanding HomeFirst Team delivering integrated holistic care spanning both acute and community settings. The post holders primary duty will be to support pathways to enhance our @home virtual ward within the National guidance, as well as provide middle grade support to the Mary Seacole Unit
The post holder will support our goal in improving the quality of care delivered to patients with a focus on maximising independence and supporting their return to their usual place of residence wherever possible. You will receive peer support from fellow doctors working across the frailty services in Epsom General Hospital and Surrey Downs Health and Care Partnership and be fully supported consultant geriatricians and an integrated management team helping you to drive forward change.

Detailed Job Description And Main Responsibilities
Main Responsibilities and Duties:
To show dynamism, enthusiasm and flexibility in participating in the supporting and shaping of community and interface frailty services as part of SDHC.
To help provide a first class clinical service to all patients.
To provide high quality leadership and support to medical staff, Nursing and Allied Health Care Professionals across the HomeFirst Service.
To lead and support the multi-disciplinary team with complex discharge planning.
To support and sustain teaching and research where possible in relevant clinical areas.
When working in the community, the successful applicant will be expected to bring core medical skills; expertise and leadership to a wider team of clinicians tasked with the delivery of multidisciplinary assessment and integrated multi-agency interaction.
To work within the framework of applicable Trust policies, procedures, and clinical governance framework.
To work within contractual requirements, and in support of, applicable Trust clinical development frameworks, business plans, and development strategies.
Person Specification Qualifications Essential Criteria
- MBBS or equivalent
- Has completed four years of training with two years in specialties relevant to geriatric medicine
Experience/ Knowledge Essential criteria
- Evidence of continuing professional development
Skills Essential criteria
- Evidence of Team working skills
- Evidence of excellent Communication skills
- Ability to make decisions autonomously, when required, but understanding when to ask for help
- Equality and Diversity Needs to have a thorough understanding of and commitment to equality of opportunity and good working relationships both in terms of day-to-day working practices
- IT Skills, Working knowledge of Microsoft Office with intermediate keyboard skills
Desirable criteria
- Project Management skills (prioritising, planning, adapting plans, resource planning)
- Management Skills and leadership skills - Must be able to prioritise own work effectively and be able to direct activities of others. Experience of managing and motivating a team
