Job description

We are seeking a motivated and compassionate Social Worker to join our specialist team, delivering high-quality social work services within community, mental health or hospital setting. This is a rewarding opportunity to make a meaningful difference by supporting individuals on their recovery journey, promoting independence, social inclusion, and positive outcomes.

In this role, you will undertake person-led assessments, risk assessments, and support planning, ensuring services are delivered in line with legislation, policy, and professional standards. You will carry out statutory social care responsibilities, assess needs, and coordinate tailored social care packages and therapeutic interventions for individuals.

You will also play a key role in supporting conditionally discharged patients, acting as a Social Supervisor by providing effective monitoring, risk management, and reporting to the Secretary of State. There may also be opportunities to deputise for the Advanced Forensic Social Worker as required.

This is an excellent opportunity for a dedicated social worker who is passionate about recovery-focused practice and committed to delivering person-led care within a dynamic and supportive multidisciplinary environment.

Main duties of the job
  • Provide direct clinical care, ensuring systems monitor and support practice and performance.
  • Promote high standards of knowledge and understanding in Forensic Social Work.
  • Act autonomously and in conjunction with the wider MDT.
  • Provide support and supervision to colleagues in the Forensic Division.
  • Provide clinical advice and leadership appropriate to professional background.
  • Work with service users and carers to facilitate collaborative engagement in individual care pathways and service development.
  • Ensure patient safety, risk reduction/mitigation and competent practice in self and others.
  • Use clinical reasoning and decision-making to support person-centred care plans.
  • Exercise professional judgement to manage risk appropriately, particularly during complex and unpredictable events, working within the team to ensure safety of individuals, families and carers.
  • Act as advocate and role model, delivering care responsive to changing requirements.
  • Support service training programmes and stakeholder training events.
  • Provide clinical support to community consultancy, including discharge preparation, positively influencing practice to enhance quality of patient-centred care.
  • Participate in debriefs following incidents and support learning from incidents.
