Job description

  • Undertaking specialist assessment and care coordination of individuals presenting with housing problems, including use of substances
  • Delivering interventions for substance use in both group and individual formats, also holding a caseload of service users
  • Providing consultation, support and advice in relation to substance use issues
  • Holding a caseload of service users, being responsible for offering a range of interventions to those individuals. Including harm reduction interventions
  • Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation.
  • Keeping up to date and accurate records and documentation.
Main duties of the job

JOB PURPOSE

To provide a dedicated, comprehensive service to people experiencing homelessness, being at risk of homelessness and having substance related problems, or being at risk of relapse back into substance use and have a history of, or are at risk of disengaging from services. To also work closely with and provide advice and guidance to colleagues within the housing options team to integrate substance use intervention into care plans, increasing engagement and ongoing support, where appropriate.

Job responsibilities

  • Provide harm reduction advice, information, and support with an emphasis on safer drug use, safer sex, HIV, Hepatitis A, B and C and other blood borne infections
  • To offer Naloxone to al clients due to risks presenting, or as a precautionary measure
  • To facilitate access to BB testing
  • Be responsible for maintaining a full case load of complex clients as the key worker
  • To keep up to date records and information for the purpose of internal and external evaluation
  • To provide information and statistics consistent with Cannock Chase District Council, Staffordshire County Council drug and alcohol commissioning team and Inclusions requirements
Person Specification
Qualifications
  • Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies or equivalent experience
Experience
  • Considerable experience of working with drug users
  • Experience of working with techniques such as CBT, Motivational interviewing, and Solution focused therapy
Skills and Abilities
  • A thorough knowledge of substances and their effects
  • Ability to work collaboratively with other professionals.
