We're looking for:o Occupational Therapistso Physiotherapistso Speech and Language Therapistso Dietitianso Registered Nurseso Support Workers across Occupational Therapy, Rehabilitation, Physiotherapy, Dietetics & Nursing

Main duties of the job

To provide an urgent 2 hour response to patients in the community to prevent hospital admission.

To undertake assessments and coordinate and provide therapy intervention to facilitate and support a safe discharge.

To undertake specialist clinical therapy, including work requiring particular expertise or ability.

To promote integrated, multidisciplinary working practices that supports seamless care.

To support the Team Lead Therapist, with service development, helping to implement policy changes and to set and monitor standards of practice.

To participate in health promotion and education of staff, clients and families
