An exciting opportunity has arisen for an enthusiastic and driven Specialist Public Health Practitioner to join our Healthy Families Team within Bassetlaw. The successful candidate will lead the Healthy Families Team in the delivery of the Healthy Child Programme in line with local and national public health priorities.

You will work as part of a team to deliver universal and targeted health services to Bassetlaw's 5-19 population and will be responsible for the day-to-day management of a defined caseload. We are continuously developing our offer to Nottinghamshire’s 5-19 population and have been drivers for change by offering a digital health questionnaire to all of our year 9 pupils as well as monthly drop-in clinics.

You will be expected to develop excellent working relationships to provide seamless services to Children, Young people, and Families. This dynamic role will provide leadership across the full scope of the Service Delivery Framework and will promote a partnership approach to health and contribute to tackling health inequalities.

This post requires a practitioner with sound communication and teamwork skills, as you will be responsible for the case management, assessment, triaging, planning, and delivery of care for parents, children, and young people using evidence-based interventions.
