This Role is a PCN Additional Role and Responsibilities (ARRS) vacancy funded by the PCN to work for Southampton Central PCN as a Specialist Proactive Care Nurse/ Paramedic (PCN). Southampton Central Primary Care Network provides services across 5 Primary Care surgeries within the centre of Southampton including Solent GP Surgery, Burgess Road Surgery, Stoneham Lane Surgery, Alma Road Surgery and Homeless Healthcare. Holding a patient list of approximately 56,000 patients.

This role involves undertaking anticipatory and person-centred care planning, leading, and supporting the delivery of anticipatory care planning for individuals at risk of hospital admission, particularly those living with frailty, complex needs or long-term conditions.

The Specialist Proactive Care Nurse/ Paramedic will be autonomous, working independently and with other health care professionals to assess and support a cohort of patients within the PCN alongside the Proactive care team to enable them to live independently. You'll provide professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

You’ll be an integral member of the practice team across all sites to support the development of new and innovative concepts, models, methods, and practices ensuring the delivery of new and improved primary care services enabling the practices to meet the needs of the population.

Interviews with be held on Thursday the 6th November at the Royal South Hants Hospital, timings TBC.

Main duties of the job
  • Develop and monitor personalised care plans
  • Early identification and clinical reviews: Supporting the early identification of patients with deteriorating health through case finding, clinical reviews, using frailty index, Q-Risk and clinical judgement to inform proactive interventions
  • Comprehensive holistic assessment
  • Co-ordinate and deliver interventions that promote self-management, resilience and independence, enabling patients to take an active role in their health and care planning. Community based care: conduct regular clinical reviews in clinical and home settings ensuring continuity and equality of access to care.
  • Health promotion and prevention: Providing education, support, and interventions related to disease prevention, lifestyle modification and health promotion.
  • Engage with communities to raise awareness of key public health messages, screening, and vaccination programmes.
  • Chronic disease monitoring and support: clinical care supporting the monitoring of chronic conditions including diabetes, hypertension, chronic kidney disease, respiratory and cardiovascular disease
  • Collaborative clinical management: Work closely with GP, practice teams, and other specialist services to optimise long term condition management, reduce complications and minimise hospital admissions
  • Evidence based care: Utilise national guidelines, protocols, and best practice frameworks to deliver safe effective and personalised care
