This Role is a PCN Additional Role and Responsibilities (ARRS) vacancy funded by the PCN to work for Southampton Central PCN as a Specialist Proactive Care Nurse/ Paramedic (PCN). Southampton Central Primary Care Network provides services across 5 Primary Care surgeries within the centre of Southampton including Solent GP Surgery, Burgess Road Surgery, Stoneham Lane Surgery, Alma Road Surgery and Homeless Healthcare. Holding a patient list of approximately 56,000 patients.
This role involves undertaking anticipatory and person-centred care planning, leading, and supporting the delivery of anticipatory care planning for individuals at risk of hospital admission, particularly those living with frailty, complex needs or long-term conditions.
The Specialist Proactive Care Nurse/ Paramedic will be autonomous, working independently and with other health care professionals to assess and support a cohort of patients within the PCN alongside the Proactive care team to enable them to live independently. You'll provide professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
You’ll be an integral member of the practice team across all sites to support the development of new and innovative concepts, models, methods, and practices ensuring the delivery of new and improved primary care services enabling the practices to meet the needs of the population.
