Overview

Job summary: A Band 6 Specialist Practitioner is sought to join our Mental Health Crisis Intervention Team. We are an urgent care service for adults aged 18-64 (and their families and carers) living in Hull and East Riding who are experiencing an acute mental health crisis. We work with partner agencies to deliver Crisis and Home Based Treatment support and gatekeeping for Inpatient care.

We are seeking professionals with a Mental Health or Learning Disability nursing registration or social worker with Mental Health experience who are keen to lead on innovation, develop robust care pathways, and enhance the patient and carer experience. The role requires motivation, enthusiasm, and experience with people who have complex needs and enduring mental illness. Strong organisational skills, prioritising and delegation abilities, and an interest in urgent and emergency mental health care are important, including providing expert clinical advice, leadership, and shift guidance.

Working pattern: We are a 24/7 service; duties include days, evenings, weekends, nights, and bank holidays.

Responsibilities
  • Assess care needs and develop and implement plans of care for individuals experiencing acute mental health deterioration or crisis.
  • Collaborate with patients, carers, and partner agencies to deliver high-quality crisis and acute interventions.
  • Provide crisis assessment and Home Based Treatment interventions, gatekeeping for inpatient admissions, coordinate the section 136 suite/Mental Health Based Place of Safety pathway at Miranda House, and support the 111 option 2 Urgent phone line.
  • Participate in partnership work with the Mental Health response Vehicle with Yorkshire Ambulance Service paramedics to provide a face-to-face response for those in mental health distress calling 111 or 999.
  • Utilise leadership and clinical advice to support and supervise on shift and ensure robust care pathways are followed.

Qualifications and Knowledge

Essential

  • Professional registration in Mental Health Nursing, Social Work (with Mental Health experience), or Learning Disabilities Nursing.
  • Full understanding of relevant policy, legislation and their application to clinical/service areas (e.g., Mental Capacity Act, Social Inclusion).
  • Full understanding/application of relevant clinical practice standards and audits.
  • Experience mentoring pre-registration students.

Desirable

  • Evidence of project work impacting practice and higher-level communication/planning/change within the area of responsibility.
  • Leadership training, qualification or experience.

Experience

  • Evidence of CPD relevant to the clinical area at specialist level.
  • Experience developing specialist practice nursing skills and demonstrating impact on practice change/development.
  • Experience promoting user/carer involvement/participation.

Desirable

  • Experience in the specific field of the post and leadership/management that positively changed service delivery/practice.
