Planned Therapy North Sefton service provides therapy and rehabilitation in patients' homes. We aim to promote independence and improve quality of life for people with physical problems caused by accidents, ageing, disease, or disability. These include respiratory conditions, orthopaedic and musculoskeletal conditions, mobility issues, neurological conditions, and those receiving palliative care.

The role involves working within the Planned Therapy North Sefton team to support our patients in their homes and community settings, providing opportunities to develop skills and advance your career. We’re a supportive team including physiotherapists, occupational therapists, assistant practitioners, and rehabilitation assistants.
The post holder will be experienced in managing complex conditions and will be expected to autonomously manage a caseload of patients with co-morbidities, using evidence-based, patient-centered principles to assess, plan, implement, and evaluate interventions. Leadership responsibilities include supervising and appraising junior staff, participating in service planning, development, and evaluation, and leading service improvement projects.
You will adopt a holistic approach in assessing and treating patients, ensuring high-quality, appropriate therapy and care for complex cases, utilizing suitable models of care and evidence-based practices.
