Cardiff and Vale University Health Board is seeking an experienced Band 7 physiotherapist to join the Hillingdon Stroke Early Supported Discharge Team on a 9-month fixed-term or secondment basis. The role combines specialist stroke physiotherapy with coordination of the ESD pathway across home visits in the London Borough of Hillingdon.

Responsibilities

  • Undertake highly specialist physiotherapy assessment and rehabilitation for people following stroke, including complex needs.
  • Provide specialist intervention relating to movement, balance, gait, transfers, spasticity, endurance and community mobility.
  • Triage referrals and support safe transfer from hospital to the ESD service.
  • Coordinate the day-to-day ESD caseload and multidisciplinary input with the Team Lead.
  • Work with acute stroke services, primary care and social care to ensure continuity of care across the pathway.
  • Provide clinical supervision, teaching and support to physiotherapists, rehabilitation staff, students and other team members.
  • Support staff to deliver safe, effective and appropriately delegated rehabilitation programmes.
  • Contribute to SSNAP data quality, audit and pathway development; promote guidelines and standards.

Job description

An exciting opportunity has arisen for an experienced and motivated Physiotherapist to join the Hillingdon Stroke Early Supported Discharge Team on a 9 month fixed-term or secondment basis. The Hillingdon Stroke Early Supported Discharge Team is a supportive and established multidisciplinary service. The team provides specialist stroke rehabilitation to people within their own homes across the London Borough of Hillingdon. The successful applicant will combine highly specialist physiotherapy practice with responsibility for supporting the day-to-day coordination of the ESD pathway. This includes managing a complex clinical caseload, coordinating referrals and transfers of care, supporting multidisciplinary caseload management, supervising staff and contributing to audit, service development and quality improvement. We welcome applications from established Band 7 physiotherapists with stroke rehabilitation experience and experienced Band 6 physiotherapists who meet the essential requirements and are ready to progress into their first Band 7 role. The successful applicant will receive a structured induction, regular supervision and a development programme tailored to their previous experience and learning needs. Applications from candidates wishing to undertake the role as a secondment are welcomed, subject to agreement with their current employer.,

  • Undertake highly specialist physiotherapy assessment and rehabilitation for people following stroke, including those with complex neurological, medical, cognitive and social needs.
  • Provide specialist intervention relating to movement, postural control, balance, gait, transfers, falls, spasticity, orthotic management, physical endurance and community mobility.
  • Triage referrals and work with hospital stroke teams to support safe and timely transfer from hospital to the ESD service.
  • Coordinate the day-to-day ESD caseload, rehabilitation priorities and multidisciplinary input in conjunction with the Team Lead.
  • Work closely with acute stroke services, the Community Rehabilitation Team, primary care, social care and voluntary-sector partners to support continuity across the stroke pathway.
  • Provide clinical supervision, teaching and support to physiotherapists, rehabilitation support workers, students and other members of the multidisciplinary team.
  • Support staff to deliver safe, effective and appropriately delegated rehabilitation programmes.
  • Contribute to SSNAP data quality, clinical audit, outcome monitoring, service evaluation and quality-improvement activity and promote practice consistent with the National Clinical Guideline for Stroke, NICE guidance, NHS England stroke-service models and local and regional stroke pathways.
  • Deputise for the Team Lead when required and contribute to the continued development of the Hillingdon community stroke rehabilitation pathway.,

The principal areas of responsibility include:
  • Highly specialist stroke physiotherapy assessment and treatment;
  • ESD referral, transfer-of-care and caseload coordination;
  • Multidisciplinary rehabilitation and case management;

  • Clinical governance, SSNAP, audit and outcome monitoring; andservice development and implementation of national and local stroke standards, Current driving licence and access to car for community visits
  • Diploma / degree in Physiotherapy
  • Current Registration with Health and Care Professions Council as Physiotherapist
  • Evidence of relevant post graduate courses/ CPD
  • Member of specialist interest group, MSc , management course or equivalent level of study, Specialist stroke experience
  • Awareness of NSF long term conditions and NICE guidelines
  • Experience of teaching, supervision and mentoring of students of physiotherapy and other AHP's
  • Experience of spasticity and orthotics management
  • Experience of splinting and specialised seating
  • Experience of audit/policy/research
  • Experience of performance management and appraisal.
  • Strong concept of case management role, Specialist community experience
  • Experience of FES (Functional Electrical Stimulation, Comprehensive clinical knowledge and skills encompassing a wide range of different complex conditions including stroke.
  • Sound assessment skills with advanced clinical reasoning and analytical ability especially with multipathology.
  • Ability to critically appraise information/ journal articles
  • Keyboard skills and ability to access information via the internet.
  • Evidence of self-directed learning and reflective practice
  • Ability to work independently and prioritize workload pressure
  • Time management skills
  • Presentation skills and the ability to pass on skills and knowledge to others (patients and staff).
  • Knowledge of developments within NHS community rehabilitation and physiotherapy profession
  • Knowledge of equipment provision, wheelchair provision and special seating needs., Current driving licence and access to car for community visits
