Sheffield Teaching Hospitals NHS Foundation Trust is seeking a physiotherapist to join their Integrated Community Stroke Service. The role involves providing specialist care within stroke recovery environments, contributing to patient assessment, and supporting continuous professional development in a collaborative team setting.
An opportunity has arisen for a physiotherapist within our integrated stroke pathway who is able to demonstrate specialist skills within stroke care. Our stroke services point of access is via a neurology assessment unit which supports rapid patient assessment and diagnosis and is collocated with a 16 bedded hyperacute service and 28 bedded acute ward. Sheffield has an integrated stroke pathway including a 30 bedded off site rehabilitation centre and community stroke services. Successful candidates will be based in the Integrated Community Stroke Service within the Sheffield Stroke Pathway.
The successful candidate will receive support, supervision and opportunities for continuing professional development within an environment that facilitates collaborative team working.
The successful post holder will work closely with other specialist physiotherapists and wider team members, evaluating and optimising the delivery of therapy in the Integrated Community Stroke Service.
The post holder will demonstrate clinical reasoning and knowledge of the hyper acute and acute stages of stroke care through to rehabilitation, inclusive of a strong awareness of patient flow, a home first principle to discharge assessment and delivering rehabilitation within an inpatient and home environment.
As a regional service experience of coordinating care across geographical boundaries is desirable alongside a strong MDT method of working to support high quality, integrated care.

Therapy Services provides a number of services over 7 days (including Saturday, Sunday, bank holidays and evenings). Successful candidates will be expected to contribute to these rosters as part of their contracted hours.
Candidates must hold a full UK driving licence and access to a car as community working and domiciliary visits are a core component of this role.
