Job summary
An exciting opportunity has arisen for a highly capable, enthusiastic and motivated pharmacist to join the Post-Acute Medicine Team. The postholder will be responsible for leading and delivering a high quality, clinical service to wards across medicine specialities working closely with the Principal Pharmacist, Post-Acute Medicine.
The successful candidate will have the opportunity to work across Post-Acute Medicine including a broad range of specialities such as respiratory, Cystic Fibrosis, acute medicine, stroke, elderly care, and diabetes. Pharmacists within the team provide extensive pharmaceutical care and comprehensive medicines management to ensure a seamless, integrated model of care.

The postholder will act as a specialist practitioner and expert pharmacist in Post-Acute Medicine, delivering pharmaceutical care and providing complex, evidence-based clinical advice to healthcare professionals inside and outside the Trust. Responsibilities include participating in ward rounds, outpatient clinics, medication reviews, and multidisciplinary meetings, as well as practicing as an independent prescriber where appropriate. The role involves ensuring timely medication supply, liaising with healthcare professionals, developing guidelines, supporting homecare services, and maintaining up-to-date clinical drug information. Participation in adverse incident reporting is also required.
