Job overview

This post offers an excellent opportunity for an enthusiastic, dynamic, and motivated pharmacist with a clinical pharmacy background to join the Post-Acute Medicine team. The post holder will lead and deliver high-quality, clinical services to post-acute medical and stroke wards.

The successful candidate will work across Post-Acute Medicine, covering specialties such as respiratory, cystic fibrosis, stroke, gastroenterology, diabetes, and elderly care. Pharmacists in this team provide extensive pharmaceutical care and medicines management to ensure a seamless, integrated model of care.

Main duties of the job

The post holder will:

  • Act as a deputy for the Principal Pharmacist when necessary.
  • Participate in multidisciplinary teams to deliver high-quality, cost-effective, patient-focused pharmaceutical care.
  • Provide interface and discharge liaison, supporting wards and units within the Care Group.
  • Lead on clinical pharmacy services within various sub-specialties, ensuring high clinical input.
  • Act as a clinical role model and supervise rotational staff at ward level.
  • Promote safe drug use within the specialty.
  • Develop, implement, and audit local guidelines.
  • Audit medicine usage to promote cost-effective prescribing.
  • Assist in financial reporting and drug budget management.
  • Ensure seamless pharmaceutical care on discharge, liaising with other healthcare professionals.
  • Work with the Principal Pharmacist to extend professional practice boundaries within the Care Group.
