This post offers an excellent opportunity for an enthusiastic, dynamic, and motivated pharmacist with a clinical pharmacy background to join the Post-Acute Medicine team. The post holder will lead and deliver high-quality, clinical services to post-acute medical and stroke wards.
The successful candidate will work across Post-Acute Medicine, covering specialties such as respiratory, cystic fibrosis, stroke, gastroenterology, diabetes, and elderly care. Pharmacists in this team provide extensive pharmaceutical care and medicines management to ensure a seamless, integrated model of care.
