Job summary

St Richard's Hospice is seeking a Specialist Palliative Care Social Worker to join the Family Support Team in Worcester. The role focuses on expert psychosocial support for patients, carers and families affected by serious illness, with opportunities to lead safeguarding and contribute to service development.

You will work within a busy multidisciplinary team, manage a varied caseload, and hold responsibilities across inpatient and community settings, delivering compassionate care and advocacy

Qualifications

• A recognised Social Work qualification.
• Current registration with Social Work England.
• Experience of working with adults and/or children with complex needs.
• Knowledge of safeguarding legislation, policy and best practice.
• Excellent assessment, communication and advocacy skills.
• Experience of multidisciplinary and partnership working.
• Ability to manage a varied workload and work autonomously when required.

Responsibilities

• Provide expert psychosocial, emotional and practical support to patients, carers and families.
• Lead safeguarding concerns and provide specialist advice to colleagues.
• Undertake psychosocial assessments, advocacy and advance care planning support.
• Facilitate individual and group support interventions.
• Collaborate with a range of professionals including health, social care, education and voluntary organisations.
• Contribute to training, service development, audit and quality improvement activities.
