Job summary

St Richard’s Hospice in Worcester is seeking a Specialist Palliative Care Social Worker to join our Family Support Team. You will provide psychosocial, emotional and practical support to patients, carers and families through serious illness, end of life, loss and bereavement, with a varied caseload on the In-Patient Unit and in the community.

• Qualified social worker with current Social Work England registration.
• Experience with adults and/or children with complex needs.
• Knowledge of safeguarding legislation and best practices.
• Ability to build therapeutic relationships in challenging times.
• Experience in multidisciplinary and partnership working.

Responsibilities

• Provide expert psychosocial support to patients, carers and families.
• Manage a diverse caseload with holistic assessments and care planning.
• Lead on safeguarding concerns and provide specialist advice.
• Facilitate individual and group support interventions.
• Collaborate with health, social care, education and voluntary organisations.
• Contribute to training, service development and quality improvement.
