Job Description:

The post holder should have the skills and experience of working and promoting Palliative Care within a Hospice and/or community or Hospital setting. Competence in the management of symptoms associated with life-threatening illness, effective communication and interpersonal skills are essential for this specialist role.

The post holder will work as a member of the Community and In-Patient based Palliative Care Team at St Margaret of Scotland Hospice and will require specialist knowledge of Palliative care. The role involves assessing and addressing the needs of patients living at home (with life-threatening illnesses) and their carers through direct/indirect care and multidisciplinary working.

1. Work closely with members of the multi-professional team within St Margaret of Scotland Hospice and the wider community projects/services to provide specialist palliative care within the Hospice and community setting.
2. Participate in daily Ward Rounds, weekly Grand Ward Rounds, and daily Patient handovers within the Specialist Palliative Care Centre St Joseph's Ward.
3. Establish yourself as a resource, facilitator, and role model for Specialist Palliative Care within the Hospice in-patient and community settings.
4. Adopt a multi-professional and collaborative team approach to care assessment and delivery to meet the needs of community-based patients and families.
5. Participate in the Hospice Virtual Clinic and/or deputise for the Community Palliative Care Lead as required.
6. Liaise with the Hospice Social Worker to optimise psychosocial aspects of the patient and family from a quality-of-life perspective.
7. Liaise with the Hospice Therapy Team to enhance the functional aspects of the patient and family from a quality-of-life perspective.
8. Support families and carers in bereavement, referring appropriately to child/young person and adult Bereavement Services if long-term support is needed.
