Job Overview

The Provide Stroke Rehabilitation Service supports 25 inpatient Stroke beds and a well established Early Supported Discharge Service. We work in partnership with the Acute Stroke Units, Social Care and the Stroke Association providing seamless, high quality stroke rehabilitation to the people in our care. Our Nursing and Therapy teams, supported by Stroke Psychologists and Medical Practitioners provide a progressive service which showcase true multi and interdisciplinary working. Our philosophy is one that encourages staff participation at all levels and identifies opportunities for service and career development.

This is an exciting opportunity to join a team of stroke skilled professionals going through a development process of increasing beds and partnership working with MSEFT to create a seamless end to end pathway for stroke patients. The post holder will be working in the Stroke Rehabilitation Unit in Brentwood, with opportunity to support in the ESD team, giving an excellent opportunity to work across the community pathway, focusing on the delivery of the ICSS model 2022.

We are looking for a motivated and dynamic Occupational therapist to join our team. If you are passionate about enabling people to regain independence and are a strong team player with a commitment to delivering the highest standard of Stroke Rehabilitation we would like to hear from you.

Main duties of the job

To work as part of an integrated multi-disciplinary team of staff within the Stroke

Rehabilitation service based at Brentwood Community Hospital.

To promote, maintain and develop a high quality, efficient and effective Occupational therapy

service to patients referred to the Mid Essex Stroke rehabilitation service, in accordance with

the philosophy and operational policies of the team.

To manage a complex caseload within stroke care using knowledge based clinical reasoning,

critical thinking, reflection and analysis to support assessments using a client centred, goal-orientated rehabilitation programme.

To be an active member of the stroke team, and participate in specific clinical projects as

agreed and supported by the Team Leader, and to participate in service developments, audit

and review of the stroke service.

To provide supervision as required to therapy assistants and occupational therapy students

whilst on placement in the service.

To contribute to induction and training for junior staff, therapy assistants, students, other

disciplines and carers

To be actively involved in Continuing Professional Development.

Detailed Job Description And Main Responsibilities

MAIN DUTIES & RESPONSIBILITIES

To manage a caseload of patients with stroke, taking a key worker role as indicated.

To set and maintain a high standard of clinical practice, which is evidence based where possible, using standardised assessment and outcome measures, where appropriate.

To complete a robust occupational therapy assessment of the patients’ need

To set person centred goals with patients and/or their carers and a treatment plan that supports this.

To monitor, evaluate and review occupational therapy interventions within a multidisciplinary framework and to modify as necessary

To have the ability, knowledge and skills required for moving and handling people in community environments.

To act as an advocate on behalf of patients and carers by adopting a person-centred approach to support meaningful participation in life roles within Mid Essex.

To work with other members of the stroke rehabilitation team, reablement, and voluntary sector to provide intervention around discharge and for post discharge rehabilitation.

To assist in and help co-ordinate the patient’s discharge planning, taking the lead role when acting as the patient’s keyworker.

To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.

To have a sound understanding of occupational therapy, National Clinical Guidelines and NICE clinical guidelines for stroke care.

To have a good understanding of neuro-anatomy, the clinical presentations of stroke and how they can impact on cognition and function.

To work with the ESD Coordinator and all the disciplines working in the team, to develop flexible interdisciplinary/multi-agency work practices which enhance outcomes for clients.

To actively be involved in service development with support and guidance from the ESD Coordinator.

To meet HCPC standards of Proficiency for occupational therapy

To keep up to date with current knowledge and advances in occupational therapy practice in stroke rehabilitation, and be responsible for own CPD in line with HCPC registration.

To contribute to and participate in the stroke in service training programme

To provide supervision, annual appraisal and monitoring the work of junior staff, therapy assistants and students with support from senior staff.

To ensure client and service records are recorded and maintained on IT databases and that client documentation is according to trust and local policies.

To provide statistics and other information as required by the service.

To comply with all policies and procedures and the Health & Safety at Work Act.

To understand and apply National Guidelines and legislation relating to occupational therapy practice.

To maintain and develop specific departmental duties as designated by the team lead.

To undertake any other duties that may be allocated.
