Are you a Band 5 Occupational Therapist looking to take the next step in your career, with the right support to build your confidence and clinical skills? An opportunity has arisen for a motivated and dynamic Occupational Therapist to join our Community Health Services team in Tower Hamlets. This is a developmental Band 5 to Band 6 role, starting in Band 5 with a structured bespoke development plan tailored to your learning needs. You will have access to regular supervision, protected learning time, and competency-based assessments, supported by experienced senior clinicians to develop clinical reasoning, risk management, and decision making in a safe, nurturing environment. Upon successful completion and sign-off of all agreed competencies, you will transition to an autonomous Band 6 role.
This developmental pathway is designed to build competence in managing more complex cases in the community. The post holder will rotate across service pathways and work with partners across health and social care to deliver holistic care in the community and avoid unnecessary hospital admissions.

This role is within the Admission Avoidance & Discharge Service, which includes an Admission Avoidance Team based in the Emergency Department, In-reach nurses, Rapid Response, and an Intermediate Care Team using a discharge-to- assess model to reduce hospital stay and admissions by supporting clients at home. The Urgent Community Response service includes Rapid Response, Falls Pickup, and Intermediate Care Teams with the aim of reducing hospital admissions and supporting patients in their own homes. The post holder will also contribute to joined-up and integrated care across health and social care interfaces.
