Role Details
The HOSM-SAS Cluster includes Seaton, Axminster, Sidmouth, Honiton, and Ottery St Mary, offering a mix of rural and coastal settings near Exeter, with excellent transport links. The location provides a healthy work-life balance with natural beauty, free parking, and flexible working options.
The ideal candidate will be interested in community rehabilitation, multidisciplinary working, and managing patients with long-term conditions such as neurological, orthopaedic, dementia, and falls prevention. The focus is on maximizing independence and quality of life, preventing hospital admissions, and supporting hospital discharges. A car and valid driving license are required.
30 hours per week, with standard hours from 8:30 to 16:30, Monday to Friday. You will also work shifts on the Ageing Well rota, covering seven days and hours from 08:00 to 20:00. We welcome applications for full or part-time roles and are open to discussing flexible working arrangements.
