Job description
Overview

We are seeking a motivated and passionate Specialist Occupational Therapist to come and join our well established Community Rehab Team.

The Specialist Occupational Therapist for Enablement and IMC will work across community settings, predominantly within the Craven Area, but also supporting the wider Community Rehab Team as required.

Responsibilities
  • The Specialist Occupational Therapist for Enablement and IMC will work across community settings, predominantly within the Craven Area, but also supporting the wider Community Rehab Team as required.
  • Providing clinical support and interventions, along with existing team members, to enable Home First discharges from Hospital Settings and provide Rehab services in Craven Intermediate care beds.
  • The community rehab team provides specialist assessment, treatment, and rehabilitation in intermediate care beds and in patients own homes.
  • This role is key in facilitating safe and timely hospital discharges local hospitals, in conjunction with Craven Enablement services as well as ensuring IMC beds are used to best effect and are supported to ensure best use of resource, To undertake a supporting role to the Team Leader managing, planning, coordinating, delivering and evaluating therapy plans as part of this broad MDT.
  • To provide assessment, treatment and rehabilitation in the home and IMC beds.
  • To act as a source of specialist knowledge and provide an advisory service to other ANHSFT therapists and members of the MDT in the management of Community Rehab patients.
  • To lead in the specialist assessment and treatment of patients within community who may have complex and chronic presentations and determine clinical diagnosis and any therapy treatment indicated and to maintain records as an autonomous practitioner.
  • To provide specialist training and education for all grades of staff and other health professionals in the therapeutic management of community rehab patients.
