To be an experienced specialist nurse with knowledge and skills in the field of plasticand dermatology surgery, undertaking holistic assessment, planning,implementation and evaluation of programmes of care in accordance with NationalGuidance.
To lead initiatives to continuously improvequality and clinical care standards.
To lead and provide seniornurse leadership for care of the patient with skin cancer or suspected skincancer.
To develop and continuouslyfocus on improving patient centred care and overall experience within the skincancer arena
Thepost holder is expected to work as part of a team and will report to the Matron,and will also work closely with all members of the Multidisciplinary team inCDDFT.
The post will support theMatron - Surgery Care Group to ensure relevant policies and guidelines forPlastic Surgery and Dermatology are developed in line with national and localrequirements.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Responsible and accountable for the managementof a patient caseload within the specialty. The greater part of the time willbe carrying out surgical procedures e.g., See & Treat sessions anddedicated lists. This could include weekend and evening working. The remainingpart of the time will be dedicated to the required managerial activities,promoting education, participation in direct care, leading and monitoring thepatient safety agenda, participation in audit to improve care delivery andpatient outcomes.
To establish and maintain effective personalrelationships with patients and relatives and ensure that the individualsphysical, psychological and spiritual needs are identified.
Ensure patient needs are met by workingcollaboratively with other professionals and agencies, especially in relationto ongoing care needs and discharge arrangements, liaising with a wide remit ofagencies/multi-disciplinary teams, hospital based and community, and has a wideknowledge of all the services offered, e.g., has the authority to admit anddischarge patients and refer to other health care providers as appropriate.
Performs a comprehensiveproblem focused health history, physical examination, analyses and interpretshistory, presenting symptoms, physical findings and diagnostic information todevelop differential diagnosis within specialist area.
Ensure patients are investigatedappropriately, interpretation of results e.g., understanding histology results,margins and depth for completion and communicate results to the patient, carerand GP/referrer. Deciding a management plan resulting in appropriate follow-up e.g.,and discharge.
Possess and demonstrateclinical skills and be able to deliver specialised nursing care, eg.,administering of local anaesthetic, perform wide local excisions, skin flapsand skin grafts. Use highly developed specialist knowledge to take InformedConsent from the patient by informing the patient and explaining the reason forthe procedure, benefits, risks, complications and effects of the intendeddiagnostic/ therapeutic surgical procedure, and any alternative treatments.
Recognise and respond appropriately to urgentand emergency situations, complete and possess Advanced Life Support skills inorder to initiate first line treatment in emergency situations e.g.,Cardiac/Respiratory arrest
Deal with highly complex clinical situations e.g.,recognizing and instigating treatment in patients experiencing an adverseevents or reaction from the nurse led procedure. e.g., bleeding,respiratory/cardiac arrest.
Frequently and directlyinvolved in the giving of bad news to patients and/or relatives, e.g.,informing patients of a cancer. Provide support to anxious and /or relatives,demonstrating empathy and sensitivity in a professional manner.
Accountable for identifying the patientshealth education needs and collaboratively implementing programmes of healthpromotion according to the diagnosis.
Participate in discharge planning, e.g.,develop specialist programmes of care and provide individual dischargeprotocols and information packages for the safe discharge of patients followingdiagnostic procedures.
Autonomously discharge patients with benignconditions who are asymptomatic with normal histological findings.
Actively monitor to ensure safe prescribingand administration of drugs in accordance with current NMC guidelines and TrustPolicies.
Discuss the future management plan with thepatient and their carer if appropriate and liaise with the referring medicalpractitioner.
Communicate and liaise withprimary care colleagues regarding patient care, management and monitoring. Beresponsible for documentation of care and treatment and subsequentletter/report to GP.
Provide highly specialist advice and supportto colleagues regarding surgical procedures across primary and secondary care.
Work autonomously to carry out complex/specialistsurgical procedures e.g., patients who are taking anticoagulants and or have apacemaker / defibrillator insitu and initiate involvement with the cardiac team/ anaesthetist ensuring patient safety.
Competence in the use of all medical devicesand specialist equipment relevant to the role, e.g., bipolar machine andsurgical instruments.
Maintain a clinical portfolio of surgicalprocedures e.g., clear margins and log in accordance with standards andguidelines.
Participate in Specialist Nurse,Multidisciplinary Team, reginal quality assurance & other meetings relevantto upholding good clinical, professional and nursing practice in themulti-disciplinary team
